TO: STEM MINDS CORP., BOREAL AGRI-INNOVATION LIMITED and CONTAINER BUILT INC. (the “Releasees”)
I, the undersigned, for myself, my dependents and guests (the “undersigned”), assume all risks and sole responsibility for any accidents, personal injury, illness, death, property loss or damages (the “Damages”) that may be sustained or incurred (a) while in, on, or about the premises municipally known as 13175 Weston Rd, King City, ON (the “Facility”) on (b) as a result of participating in the Program (defined below), and do hereby fully and forever release and discharge the Releases, their employees, shareholders, officers, directors, agents, and independent contractors from any and all suits, claims, damages, costs and expenses of every nature and kind, including Damages incurred in connection with the undersigned’s participation in the Program or its use of any equipment at the Facility except to the extent arising out of the sole negligence or willful misconduct of any of the Releasees.
The undersigned further agrees to use all equipment, all parts of the Facility in accordance with direction provided and/or solely for their permitted use and to leave them in good condition.
The undersigned shall not make any claim or take any proceeding against the Releasees or against any person which might claim contribution or indemnity from any Releasee.
The undersigned confirms that (a) it wishes to participate in the Skills Development and Training Program Agri-Tech and Agri-Food (the “Program”) that will be carried on at the Facility; (b) it understands the nature of the Program and the activities involved in the Program and has evaluated the risks inherent in participating in the Program; and (c) all questions relating to the Program have been answered to its satisfaction.
The undesigned certifies that the information on its registration form for the Program is complete and accurate.
